Are you accepting new clients? How soon will I get an appointment?
My schedule typically books about 2-3 weeks out. Appointments are available Mon-Thu 12:30-7:30pm in office or by video. I have both short- and long-term clients, so a spot will usually open up in a few weeks. Consultations are available on a limited basis, so it is best to schedule an intake session to see if we would be a good fit. Due to practice policy, discussions about goals, treatment, and symptoms occur after the necessary consents are signed for a full intake session. After completing the intake request form, you will then receive an email invitation to the client portal to complete paperwork and schedule your first appointment. Please note that for minor children, both parents/legal guardians are required to complete and sign intake forms.
Do you take insurance?
I am in-network with most employer-based plans through BCBS, Aetna (including Banner Aetna), Cigna, and UHC/Optum.
Please be sure to check that services are covered by insurance. I may not be in-network if your insurance is managed by a third party (e.g., EMI Health). I am unable to bill most secondary insurance plans.
I do not accept Medicaid/AHCCCS/ALTCS, Medicare or Tricare.
How much will I pay if I use insurance?
I have no idea :-) Your benefits are specific to you, so you will have to call your insurance company to find out if you need to meet your deductible, pay a copay, etc. You can try using Alma's insurance estimate if you have UHC, Aetna or Cigna: https://secure.helloalma.com/check-my-insurance-coverage/. If it doesn't work, there is a chance I may not be in your network. I am in-network with BCBS, however, eligibility is obtained through another method.
How do you handle payments?
Clients are asked to keep a valid credit card or debit card on file and enroll in autopay to cover any copays and any unexpected cancellation fees. You may request to be invoiced to use your HSA, FSA or HRA debit card to pay for co-pays, co-insurance or session fees after services are rendered.
What is your cancellation policy?
Please try to give as much notice as possible if you need to cancel or reschedule your appointment. There is a $75 cancellation fee for any appointment canceled or rescheduled with less than 24 hours. I understand that situations come up but it is difficult to fill slots on short notice. If you want to change from office to video session for the same day/time, I will waive the cancellation fee.
Please note you cannot bill insurance or use your HSA, FSA or HSA benefits to cover cancellation/no show fees.
How often are sessions? How long will I/my child be in therapy?
Sessions usually start off weekly or every other week. As we make progress and meet goals, we'll likely move to about once a month. The length of treatment can vary depending on many factors, and we can discuss that together to find what works best for you.
Are there services not provided?
I don't provide evaluations for autism and/or ADHD diagnosis. However, I'm happy to provide referrals if needed. I also follow AZBBHE policy, so I don't offer evaluations for fitness/duty for work, letters for Emotional Support Animals, or fill out FMLA paperwork. If you need support in these areas, please reach out to your PCP or a psychiatrist.
