Are you accepting new clients? How soon will I get an appointment?
I have both short- and long-term clients, so a spot will usually open up in a few weeks. New clients can usually get an appointment within 2-3 weeks. Appointments are available Mon-Thu 12:30-7:30pm and availability can vary weekly. Request a child intake or an adult intake.
Do you take insurance and how much will it cost?
I am in-network with most employer-based plans through BCBS, Aetna (including Banner Aetna), Cigna, and UHC/Optum. Your benefits are specific to your plan, so you’ll need to contact your insurance company to find out whether you have a deductible, copay, or coinsurance. I may not be in‑network if your plan is managed by a third party (e.g., EMI Health). I am unable to bill most secondary insurance plans.
I do not accept Medicaid/AHCCCS/ALTCS, Medicare or Tricare.
I have an authorization from my EAP
Please be sure your authorization letter/email includes your name, number of sessions authorized, date of referral, and lists me as the counselor. If you only have an authorization number, the sessions may not be certified yet, and you will need to call the EAP company to complete that process. If you have a certified authorization, please text or email me so we can schedule an appointment.
How do you handle payments?
Clients are asked to keep a valid credit card or debit card on file and enroll in autopay to cover any copays and any unexpected cancellation fees. I bill some insurance through a third party, so you are required to keep a payment on file in both systems. If you use an HSA/FSA card, you may be asked for a secondary payment method, as these cards cannot be billed for cancellation fees.
What is your cancellation policy?
Please give as much notice as possible if you need to cancel or reschedule. There is a $75 fee for any appointment canceled or rescheduled with less than 24 hours’ notice. I understand that things come up, but it is difficult to fill appointments on short notice. If you want to switch from office to video for the same day and time, the cancellation fee is waived.
Insurance, HSA, FSA, and HRA funds cannot be billed for cancellation or no‑show fees.
How often are sessions? How long will I/my child be in therapy?
Most clients start weekly or every other week. As progress is made, we often shift to monthly sessions. The frequency and length of treatment varies depending on your goals, needs, and circumstances. We’ll discuss and make a plan for what makes the most sense for you.
Are there services not provided?
I do not provide evaluations for autism or ADHD diagnosis, though I’m happy to offer referrals.
In accordance with AZBBHE policy, I do not provide fitness‑for‑duty evaluations, letters for Emotional Support Animals, or complete FMLA paperwork. For these needs, please contact your PCP or a psychiatrist.
